Nonoxynol 9 Spermicide Contraceptive - Use, Side effects, Brands

Nonoxynol 9 is a surfactant and is present in most cosmetic formulations. It is commonly used as a spermicidal agent when used intravaginally. Thus, it is used as a locally acting non-hormonal contraceptive agent.

  • Encare 100 mg:

    • At least 10 mins prior to intercourse, unwrap and insert 1 suppository vaginally.
    •  1-hour effectivity.
  • Options Conceptrol Gel 4 percent:

    • Insert 1 applicatorful (100 mg) vaginally immediately prior to intercourse
    • 1-hour effectivity.
  • Options Gynol II gel 3 percent:

    • Insert 1 applicatorful (150 mg) vaginally immediately prior to intercourse.
    • 1-hour effectivity.
    • Apply 1 applicatorful (approximately 1 teaspoonful) of gel onto the dome of the diaphragm and spread around the edge using fingertip prior to inserting, if used with a diaphragm.
    • Additional application of gel is needed, if intercourse occurs >6 hours after insertion, or if repeated intercourse occurs.
    • To reapply, Do not remove the diaphragm.
    • Use the applicator to apply the gel, taking care not to dislodge the diaphragm.

Pregnancy Implications:

  • The use of spermicides has not been associated with birth defects.
  • In the prevention of pregnancy, the effectiveness of available nonoxynol 9-containing products is similar.
  • However, the percentage of unintended pregnancies following the use of these products is generally greater than with other methods of contraception.
  • Many can be used with a diaphragm & male condom.
  • To reduce the risk for sexually transmitted disease and HIV transmission, consistent and correct use of the male latex condom is recommended and provides an additional method of contraception.
  • For use in women, barrier contraceptives may not be appropriate in which the risk of pregnancy is unacceptable due to other health conditions, because of their relatively higher failure rate if not used correctly.

  • If allergic to nonoxynol 9 or any component of the formulation.
  • If allergic to sulfites.
  • Within 6 weeks after giving birth.
  • History of toxic shock syndrome.
  • During the menstrual period.

Warnings and Precautions

  • Toxic shock syndrome:

    • Has been reported with barrier contraceptive use.
  • Vaginal irritation:

    • May cause vaginal irritation (eg, burning, itching, or rash).
    • Irritation may be associated with an increased risk of getting HIV from an infected partner.

How to administer Nonoxynol 9?

Intravaginal:

  • For vaginal use only.
  • Not for rectal use.

Today:

  • Prior to use, wet the sponge with clean water and squeeze gently until sudsy in order to activate spermicide.
  • Fold sponge in half with dimple side inside and string loop on the bottom end, then insert into the vagina.
  • Prior to intercourse, it may be inserted up to 24 hours.
  • If it accidentally falls out, Sponge should not be reinserted  (insert new sponge).
  • Do not reuse sponge.

VCF:

  • Film:
    • Before touching film, fingers should be dry.
    • Remove film from pouch & fold in half.
    • Using a finger, insert vaginally.
  • Foam:
    • Prior to use, shake well.

  • Nonoxynol 9 is a nonionic surfactant which prevents pregnancy by damaging the cell membrane of sperm.
