Plasma Protein Fraction (Biseko, Plasmanate) - Uses, Dose, Side effects

Plasma Protein Fraction is a relatively less purified form of plasma proteins derived from human plasma that contains albumin and plasma globulins. It is usually given to patients with hypotension and shock as a result of plasma leakage.

Plasma protein fraction Uses:

  • Shock:
    • It is indicated for the treatment of shock resulting from loss of plasma fluids and proteins (as in shock following severe burns, crush injuries, and abdominal emergencies).
    • It may be administered to patients with shock secondary to hemorrhage.

Plasma Protein Fraction Dose in Adults

Plasma Protein Fraction during Shock:

  • The usual minimum dose is 250 to 500 mL (12.5 to 25 g of protein) intravenously;
  • Adjust the dose based on the clinical response.

Biseko use in Children:

It is usually administered in a dose of 1 - 2 ml/kg body weight.   

Pregnancy Risk Factor C

  • There have never been any studies on human or animal pregnancies.
  • It is best to avoid volume expansion in pregnancy unless it is strongly advised.

Dose in Kidney Disease:

There are no dosage adjustments provided in the manufacturer's labeling.   

Dose in Liver disease:

There are no dosage adjustments provided in the manufacturer's labeling.   

Side effects of Plasma Protein Fraction:

  • Cardiovascular:
    • Edema
    • Flushing
    • Hypotension (related to rate of infusion)
  • Central nervous system:
    • Headache
  • Dermatologic:
    • Urticaria
  • Gastrointestinal:
    • Nausea
  • Neuromuscular & skeletal:
    • Back pain
  • Respiratory:
    • Pulmonary edema

Contraindications to Plasma protein Fraction:

  • Hypersensitivity to plasma proteins and any component of the formulation
  • Patients receiving cardiopulmonary bypass
  • Severe anemia
  • heart failure;
  • An increase in blood volume

Warnings and precautions

  • Hypotension
    • Hypotension may occur when intravenous injections are given rapidly.
    • This may lead to the need for temporary withholding or decreasing the rate of infusion.

Monitoring parameters:

  • Blood pressure,
  • pulse,
  • pulmonary exam

How to administer Plasma protein fraction?

  • It is available for IV administration only.
  • The infusion must be administered within four hours of preparing it.
  • Rapid infusions should be avoided to prevent volume overload.
  • Rapid infusions (exceeding 10 ml/minute) may result in hypotension.
  • The rate of infusion must be reduced to avoid hypotension.   

Mechanism of action of Plasma protein Fraction:

It is derived from human plasma to increase blood volume.
