Tioconazole (Trosyd Cream) is a broad-spectrum antifungal drug that is applied inside the vagina to treat local fungal infections. It is used to treat vaginal candidiasis.

Tioconazole (Trosyd Cream) Uses:

  • Vulvovaginal Candidiasis, :

    • It is indicated for the local treatment of vulvovaginal candidiasis.

Tioconazole (Trosyd) Dose in Adults

Tioconazole (Trosyd) Dose in the treatment of vulvovaginal candidiasis:

  • Intravaginal: Insert one applicatorful in the vagina at bedtime once daily.

Use in Adolescents:

Refer to adults’ dose.

Pregnancy Risk Category: C

  • If the drug is used in the vaginal area, a small amount may be absorbed.
  • A single-dose regimen should not be used for pregnant women suffering from vulvovaginal candidiasis.
  • It is recommended to follow a 7-day treatment plan. 
  • Treatment may also cause diaphragm and latex condom weakness. 
  • Patients who don't want to become pregnant should be cautious.

Dose in Kidney Disease:

  • No dosage adjustments have been provided in the manufacturer's labeling.
  • However, an adjustment in the dose is not required due to low systemic absorption.

Dose in Liver disease:

  • No dosage adjustments have been provided in the manufacturer's labeling.
  • However, an adjustment in the dose is not required due to low systemic absorption.

Antifungal Agents (Vaginal) may diminish the therapeutic effect of Progesterone.

How to administer Tioconazole (Trosyd)?

Intravaginal:

  • It is intended for intravaginal use only. One prefilled applicator is inserted intravaginally.
  • The applicator is inserted gently into the vagina without causing any discomfort.
  • It is inserted as far as possible in a comfortable position.
  • Press the plunger until it stops and then withdraw the applicator.
  • Patients should be advised not to use intravaginal products such as tampons and douches or engage in vaginal intercourse during the treatment. The applicator is discarded after use.

Mechanism of action of Tioconazole (Trosyd):

  • It is a 1-substituted imidazole derivative. 
  • It is active against many dermatophytes as well as yeasts and has a broad-spectrum antifungal activity. 

These include:

  • Trichophyton mentagrophytes,
  • T. rubrum
  • T. erinacei,
  • T. tonsurans
  • Microsporum canis,
  • Microsporum Gypseum,
  • Candida albicans.

Both agents seem to be equally effective against Epidermophyton fluccosum.

Onset of action:

  • Some improvement in symptoms occurs within one day of treatment. Complete relief may take up to a week.

Absorption following intravaginal use:

  • Systemic 10.6 to 35.8 ng/mL at 8 hours postdose;
  • none detectable at 24 hours.

Distribution:

  • Vaginal fluid: 24 to 72 hours.
