Ciclopirox olamine for the treatment of skin conditions

Ciclopirox (olamine) disrupts the DNA synthesis of fungal cells. It is used for the following Dermatologic conditions (infectious and seborrheal):

Cream and suspension:
- For the topical treatment of:
  - Tinea pedis, Tinea cruris, and Tinea corporis due to
    - Trichophyton rubrum,
    - Trichophyton mentagrophytes,
    - Epidermophyton floccosum, and Microsporum canis
    - Candidiasis (moniliasis) due to Candida albicans
    - Tinea (pityriasis) versicolor due to Malassezia furfur.
Gel:
- For the topical treatment of seborrheic dermatitis of the scalp and interdigital tinea pedis and tinea corporis due to:
  - T. rubrum
  - T. mentagrophytes, or
  - E. floccosum
Nail lacquer topical solution:
- For the topical treatment of immunocompetent patients with mild to moderate onychomycosis of fingernails and toenails, without lunula involvement, due to:
  - Trichophyton rubrum
Shampoo:
- For th topical treatment of seborrheic dermatitis of the scalp in adults.

Ciclopirox Dose in Adults

Ciclopirox treatment in patients with Tinea pedis and tinea corporis:

Topical (Cream, gel, and suspension):
- Gently massage into affected areas and surrounding skin twice daily.

Dose in Tinea cruris, cutaneous candidiasis, and tinea versicolor:

Topical (Cream and suspension):
- Gently massage into affected areas and surrounding skin twice daily.

Onychomycosis of the fingernails and toenails:

Topical Lacquer (solution):
- Apply to adjacent skin and affected nails daily. Remove with alcohol every 7 days.

Seborrheic dermatitis (scalp):

Topical: Gel:
- Gently massage into the affected area twice daily.
Shampoo:
- Apply about 5 mL to wet hair
- Lather, and leave on hair and scalp for about 3 minutes and then rinse (may use up to 10 mL for longer hair).
- Repeat twice weekly for 4 weeks.
- Allow a minimum of 3 days between applications.

Ciclopirox Dose in Childrens

Ciclopirox in Cutaneous candidiasis, tinea cruris, and tinea versicolor:

Cream and suspension (lotion):
- Children older than10 years and Adolescents:
  - Apply twice daily (morning and evening) to the affected area

Use in patients with Tinea corporis and tinea pedis:

Cream and suspension (lotion):
- Children older than 10 years and Adolescents:
  - Apply twice daily (morning and evening) to the affected area
Gel:
- Adolescents older than 16 years of age:
  - Apply twice daily in the morning and evening to the affected areas and surrounding skin.

Onychomycosis of the fingernails and toenails:

Lacquer (solution):
- Children older than 12 years of age and Adolescents:
  - Apply to adjacent skin and affected nails once daily preferably at bedtime or 8 hours before washing. Remove with alcohol every 7 days.

Seborrheic dermatitis of the scalp:

Adolescents older than 16 years of age:
- Gel:
  - Gently massage into affected areas and surrounding skin twice daily
- Shampoo:
  - Apply about 5 mL to wet hair, lather, and leave in place for about 3 minutes then rinse.
  - May use up to 10 mL for longer hair.
  - Repeat twice weekly for 4 weeks.
  - Allow a minimum of 3 days between applications

Animal reproduction studies have not shown any adverse events.

Ciclopirox use during breastfeeding

It is unknown if ciclopirox can be found in breastmilk. Nursing mothers should use it with caution.

Ciclopirox Dose in Renal Disease:

No dose adjustments has been recommended by the manufacturer in patient with renal disease.

Ciclopirox Dose in Liver Disease:

No dose adjustments has been recommended by the manufacturer in patient with liver disease.

Local:
- Application site burning,
- Local irritation,
- Local pain
Ophthalmic:
- Eye pain

Contraindication to Ciclopirox include:

Allergies to ciclopirox and any component of this formulation

Warnings & Precautions

Irritation
- If irritation or severe itching is caused by its use, discontinue using it and consult your doctor immediately.
Use it in the right way:
- You should only use it for topical purposes. Avoid contact with eyes and mucous membranes. Avoid the use of occlusive dressings and wrappings.

Monitoring Parameters:

Not recommended.

How to use Ciclopirox?

Ciclopirox should be used for topical purposes only. It should not be administered in the eyes, taken orally or used for intravaginal purposes.

Ciclopirox Cream:
- Massage gently into affected areas and surrounding skin in the morning and evening.
Gel:
- Massage gently massage into clean, affected areas or to the scalp and adjacent skin in the morning and evening.
Lacquer (solution):
- Apply evenly over the entire nail plate, nail bed and under the nail plate and over the surrounding skin at bedtime (or allow for 8 hours before washing)
- Apply the solution daily over the previous coat for 7 days and then remove the coat with alcohol.
- Every 7 days after ciclopirox is removed with alcohol, file away (with an emery board) loose nail material and trim nails, as required or as directed by a health care professional.
Shampoo:
- Wet hair and apply ciclopirox to the scalp, lather and leave on hair and scalp for 3 - 5 minutes and then rinse. Avoid contact with eyes.
Suspension:
- Shake well before use. Gently massage into affected areas and surrounding skin in the morning and evening.

Mechanism of action of Ciclopirox:

Ciclopirox inhibits the transport of essential elements and disrupts the synthesis DNA,RNA, and protein in fungal cells.
