Terconazole cream/ suppository - Drug Details

Terconazole is an antifungal medicine that is administered intravaginally as a topical agent.

It is used to treat patients with vulvovaginal candidiasis (moniliasis).

Since terconazole is effective only against the genus Candida, a KOH smear or culture should be performed prior to the local use of terconazole.

Terconazole Dose in Adults

Terconazole dose in the treatment of vulvovaginal Candidiasis:

  • Vaginal cream 0.4%:

    • Insert one applicatorful intravaginally at bedtime for seven days.
  • Vaginal cream 0.8%:

    • Insert one applicatorful intravaginally at bedtime for three days.
  • Vaginal suppository:

    • Insert one suppository intravaginally at bedtime for three consecutive days.

Terconazole Dose in Childrens

Not recommended for use in children

Pregnancy Risk category: C

  • Animal studies have shown adverse fetal events.
  • The CDC recommends that topical azole-antifungals be used during pregnancy for a maximum of 7 days.
  • The manufacturer suggests that you use it with caution, particularly during the first trimester.
  • The product can also weaken the latex in condoms or diaphragm.

Terconazole use during breastfeeding:

  • Its secretion into breastmilk is not well understood.
  • Manufacturers recommend weighing the risks and benefits to mother and foetus.

The manufacturer has not recommended any dose adjustment in patients with kidney disease.

Warnings and precautions

  • Dermatologic toxicities:
    • Patients who develop toxic epidermal necrolysis (TEN) should discontinue the treatment.
    • It is important to provide supportive care and monitoring regularly.
    • The treatment should not be recommenced.
  • Hypersensitivity and anaphylaxis
    • Patients suffering from anaphylaxis must stop receiving treatment and should be provided with supportive care and regular monitoring.
    • Reinitiation should not be encouraged.
  • Reactions that are unusual:
    • It should be stopped if you experience fever, chills, or flu-like symptoms.
    • Reinitiation should not be encouraged.
  • Irritation
    • If you have allergic reactions or irritation, discontinue using it.
    • Restart should be discouraged

Antifungal Agents (Vaginal) may diminish the therapeutic effect of Progesterone.

How to administer Terconazole?

  • Vaginal cream:

    • The applicator provided by the manufacturer should be used.
    • Insertion into the vagina should be as far as possible without causing discomfort.
    • The applicator should be washed after each use and allowed to dry before putting it back together.
  • Vaginal suppository:

    • Before using it, remove the foil package.
    • Insert the suppository into the vagina as far as possible without causing discomfort.
    • If the applicator provided is used for insertion, it should be thoroughly washed and dried before using it again.

Mechanism of action of Terconazole:

  • Terconazole can be used as an antifungal agent. It is a triazole-ketal antifungal, which inhibits the fungal Cytochrome P450 enzyme.
  • Specifically, it inhibits the cytochrome P 450-dependent 14-alpha-demethylase that results in the accumulation of membrane disturbing 14-alpha-demethylsterols and ergosterol depletion.

Absorption: Suppository: about 70% remains in vaginal area; about 10% is absorbed systemically

Time to peak, serum: about 5 - 10 hours

Half-life elimination: 6.4 - 8.5 hours
